Professional:MONITOR: Coadministration with conivaptan may increase the serum concentrations of digoxin. The proposed mechanism is conivaptan inhibition of the P-glycoprotein-mediated intestinal or renal efflux of digoxin. When a single 0.5 mg dose of digoxin was administered with oral conivaptan 40 mg twice daily, digoxin peak concentration (Cmax) and systemic exposure (AUC) increased by approximately 79% and 43%, respectively, while clearance decreased by 30%.
MANAGEMENT: Serum digoxin levels and pharmacologic effects should be closely monitored following the addition or withdrawal of conivaptan, and the digoxin dosage adjusted as needed. Patients should be advised to seek medical attention if they experience signs of digoxin toxicity such as nausea, anorexia, visual disturbances, slow pulse, or irregular heartbeat.
