MONITOR: Orlistat may reduce the bioavailability of oral amiodarone and its active metabolite, N-desethylamiodarone. The proposed mechanism is alteration of the gastrointestinal lipid phase due to inhibition of dietary fat absorption by orlistat, which may decrease the absorption of highly lipophilic drugs. When a single 1200 mg dose of amiodarone was administered to healthy study subjects following treatment with orlistat 120 mg three times daily for 13 days, systemic exposure to amiodarone and N-desethylamiodarone decreased by 23% to 27%. The effect of initiating orlistat treatment in patients on stable amiodarone therapy has not been studied.

MANAGEMENT: Caution is advised when amiodarone is used with orlistat. The possibility of diminished therapeutic effects of amiodarone should be considered. Patients should be monitored more closely following the addition or withdrawal of orlistat, and the amiodarone dosage adjusted as needed.
