Coumadin and Mitotane

Using warfarin together with mitotane can decrease the effects of warfarin. Call your doctor promptly if you have any signs of blood clots such as chest pain, shortness of breath, sudden loss of vision, or pain, redness or swelling in an extremity. You may need a dose adjustment in addition to testing of your prothrombin time or International Normalized Ratio (INR). It is important that you tell your healthcare provider about all other medications that you are using including vitamins and herbs. Do not stop using your medications without talking to your doctor first.

MONITOR: Mitotane may decrease the effectiveness of orally administered anticoagulants. The mechanism is induction of hepatic microsomal enzymatic clearance. Data are available for warfarin.

MANAGEMENT: Close monitoring for clinical and laboratory evidence of altered anticoagulant efficacy is recommended. The dosage requirements for orally administered anticoagulants are expected to increase after mitotane is added. The clinician should be also be aware that the risk of bleeding from over-anticoagulation is increased when mitotane is withheld.
