Relevance: 01.06.2022 Reviewer: Shkutko P.M., M.D., in

In the database of official manuals used in the service creation an interaction registered by statistical results of studies was found, which can either lead to negative consequences for the patient health or strengthen a mutual positive effect. A doctor should be consulted to address the issue of joint drug administration.

Consumer:

Obeticholic acid may reduce the blood-thinning effects of warfarin. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: Coadministration with obeticholic acid may diminish the hypoprothrombinemic effects of warfarin. The mechanism of interaction has not been described. When a single 25 mg dose of warfarin was administered with obeticholic acid 10 mg once daily, systemic exposure to the biologically more active S(-) enantiomer of warfarin increased by 13%, but maximum INR decreased by 11%.

MANAGEMENT: The International Normalized Ratio (INR) should be monitored more closely following initiation, discontinuation, or change of dosage of obeticholic acid, and the warfarin dosage adjusted as necessary. The same precaution may be applicable during therapy with other vitamin K antagonists, although clinical data are lacking.
