Coumadin and Tasigna

Nilotinib may alter the blood levels and effects of warfarin. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Call your doctor promptly if you experience any unusual bleeding or bruising, swelling, vomiting, blood in your urine or stools, headache, dizziness, or weakness. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Based on in vitro data, coadministration with nilotinib may alter the plasma concentrations of warfarin. Nilotinib has been shown to be a competitive inhibitor as well as inducer of CYP450 2C9, of which warfarin is a substrate. In healthy volunteers, single-dose administration of nilotinib did not change the pharmacokinetics and pharmacodynamics of warfarin. However, the ability of nilotinib to inhibit or induce metabolism has not been determined in vivo.

MANAGEMENT: Caution is advised if warfarin is used in combination with nilotinib. The INR should be checked frequently and warfarin dosage adjusted accordingly, particularly following initiation, discontinuation or change of dosage of nilotinib in patients who are stabilized on their warfarin regimen. Patients should be advised to promptly report any signs of bleeding to their physician, including pain, swelling, headache, dizziness, weakness, prolonged bleeding from cuts, increased menstrual flow, vaginal bleeding, nosebleeds, bleeding of gums from brushing, unusual bleeding or bruising, red or brown urine, or red or black stools.
