Consumer:

Venetoclax can increase the blood levels of warfarin and cause you to bleed more easily. You may need a dose adjustment or more frequent monitoring of your prothrombin time or INR by your doctor to safely use both medications. You should seek immediate medical attention if you experience any unusual bleeding or bruising, or have other signs and symptoms of bleeding such as dizziness; lightheadedness; red or black, tarry stools; coughing up or vomiting fresh or dried blood that looks like coffee grounds; severe headache; and weakness. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with venetoclax may increase the plasma concentrations of warfarin. The mechanism of interaction has not been described. In 3 healthy study subjects given a 5 mg dose of warfarin with a 400 mg dose of venetoclax, warfarin peak plasma concentration (Cmax) and systemic exposure (AUC) increased by 18% and 28%, respectively. The magnitude of interaction may be greater during chronic administration of venetoclax to steady-state.

MANAGEMENT: Caution is advised when warfarin is used concomitantly with venetoclax. The INR should be closely monitored, particularly following initiation, discontinuation, or change of dosage of venetoclax. Patients should be advised to promptly report any signs of bleeding to their physician, including pain, swelling, headache, dizziness, weakness, prolonged bleeding from cuts, increased menstrual flow, vaginal bleeding, nosebleeds, bleeding of gums from brushing, unusual bleeding or bruising, red or brown urine, or red or black stools.
