Venetoclax can increase the blood levels of warfarin and cause you to bleed more easily. In 3 healthy study subjects given a 5 mg dose of warfarin with a 400 mg dose of venetoclax, warfarin peak plasma concentration (Cmax) and systemic exposure (AUC) increased by 18% and 28%, respectively. The magnitude of interaction may be greater during chronic administration of venetoclax to steady-state.

MANAGEMENT: Caution is advised when warfarin is used concomitantly with venetoclax. The INR should be closely monitored, particularly following initiation, discontinuation, or change of dosage of venetoclax. Patients should be advised to promptly report any signs of bleeding to their physician, including pain, swelling, headache, dizziness, weakness, prolonged bleeding from cuts, increased menstrual flow, vaginal bleeding, nosebleeds, bleeding of gums from brushing, unusual bleeding or bruising, red or brown urine, or red or black stools.
