Talk to your doctor before using melatonin together with warfarin. Melatonin has been reported to increase the effects of warfarin in some patients. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. You should seek immediate medical attention if you experience any unusual bleeding or bruising, or have other signs and symptoms of bleeding such as dizziness; lightheadedness; red or black, tarry stools; coughing up or vomiting fresh or dried blood that looks like coffee grounds; severe headache; and weakness. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Limited data suggest that melatonin may potentiate the effects of warfarin. The precise mechanism of interaction has not been established. The World Health Organization's Uppsala Monitoring Centre has documented six cases of suspected interaction, but detailed information regarding each case is unavailable. According to a Cochrane review that referenced the cases, reduced prothrombin was reported in four of the cases, with bleeding complications (eye hemorrhage, purpura, nosebleed) occurring in two. No details were provided for the remaining two cases, including how prothrombin was affected.

MANAGEMENT: Patients treated with warfarin may require more frequent monitoring of their INR following initiation or discontinuation of melatonin.
