Using levothyroxine together with indinavir may increase the effects of levothyroxine. Contact your doctor if you experience nervousness, palpitations, restlessness, weakness, and weight loss. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: A single case report suggests that indinavir may increase serum thyroxine concentrations, potentially resulting in hyperthyroidism. The proposed mechanism is indinavir inhibition of glucuronosyltransferase, which is partially responsible for the metabolic clearance of thyroxine. The case report describes an HIV-infected patient who had been stabilized on levothyroxine 0.75 mg/day for several years when treatment with stavudine, lamivudine, and indinavir was begun. Approximately 50 days later, she presented with nervousness, palpitations, restlessness, weakness, and slight weight loss. Thyroid function testing revealed suppression of thyroid-stimulating hormone (TSH less than 0.1 mIU/L), increased free T4 (57 pmol/L), and increased free T3 (33 pmol/L). Thyroid function returned to normal one month after the levothyroxine dose was reduced to 0.12 mg/day.

MANAGEMENT: During coadministration with indinavir, the possibility of an enhanced therapeutic response to levothyroxine should be considered. Dosage adjustment as well as clinical and laboratory monitoring may be appropriate whenever indinavir is added to or withdrawn from therapy.
