If you are currently being treated with warfarin, you should let your doctor know before you start taking liothyronine. Increasing your thyroid hormone levels can sensitize you to the effects of warfarin, which may increase your risk of bleeding. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. On the other hand, no special precautions are necessary if you have been stable on liothyronine therapy and are now starting treatment with warfarin. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Since thyroid hormones may increase the catabolism of vitamin K-dependent clotting factors, the hypoprothrombinemic response to oral anticoagulants may be enhanced during the initiation of thyroid hormone therapy. No significant effects are expected when warfarin is added to a patient stabilized on thyroid replacement therapy.

MANAGEMENT: INR or prothrombin time should be monitored closely when initiating, discontinuing, or changing the dosage of thyroid hormone therapy in patients stabilized on their oral anticoagulant regimen. Patients should be advised to notify their physician if they develop signs and symptoms of excessive anticoagulation, such as unusual or prolonged bleeding, bruising, vomiting, change in stool or urine color, headache, dizziness, or weakness.
