Ask your doctor before using lopinavir together with darunavir. Using these medications together may decrease the effects of darunavir. Contact your doctor if your condition or infection worsens. If your doctor prescribes these medications together, you may need a dose adjustment or special tests to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

GENERALLY AVOID: Coadministration with lopinavir/ritonavir may decrease the plasma concentrations of darunavir, even in the presence of low-dose ritonavir as a pharmacokinetic booster. The mechanism of interaction has not been described. In nine study subjects, administration of darunavir/ritonavir (300 mg/100 mg twice a day) with lopinavir/ritonavir (400 mg/100 mg twice a day) decreased the peak plasma concentration (Cmax), area under the concentration-time curve (AUC) and trough plasma concentration (Cmin) of darunavir by 39%, 53% and 65%, respectively, compared to administration without lopinavir/ritonavir. In contrast, lopinavir Cmax, AUC, and Cmin increased by 22%, 37%, and 72%, respectively, in the presence of darunavir/ritonavir.

MANAGEMENT: The appropriate dosage of darunavir when prescribed in combination with lopinavir/ritonavir has not been established. Given the risk of reduced viral susceptibility and resistance development associated with subtherapeutic antiretroviral drug levels, the use of darunavir with lopinavir/ritonavir is not recommended, with or without an additional low-dose of ritonavir.
