Ask your doctor before using darunavir together with a rifapentine. Using these medications together can make darunavir less effective. Contact your doctor if your infection or condition worsens. You may need a dose adjustment or special tests to safely take both medications together. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

GENERALLY AVOID: Coadministration with rifapentine may decrease the plasma concentrations and antiretroviral effects of protease inhibitors. The mechanism is rifapentine induction of CYP450 3A4, the isoenzyme responsible for the metabolic clearance of protease inhibitors. Rifapentine has been shown to substantially increase the clearance of indinavir when coadministered at their normally recommended dosages. Indinavir peak plasma concentration (Cmax) and systemic exposure (AUC) decreased by 55% and 70%, respectively, while rifapentine pharmacokinetics were unaffected. No data are available concerning the use of rifapentine with other protease inhibitors. However, the interaction is also expected to occur based on their similar metabolic profile to indinavir.

MANAGEMENT: Given the risk of reduced viral susceptibility and resistance development associated with subtherapeutic antiretroviral drug levels, the use of rifapentine should be avoided in patients receiving protease inhibitors.
