Consumer:

Using primidone together with divalproex sodium may increase the effects of primidone. Contact your doctor if you experience slow or shallow breathing, blurred vision, extreme drowsiness, urinating less than usual or not at all, feeling light-headed, or fainting. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: Valproate may increase the plasma levels of primidone's major metabolite, phenobarbital. The mechanism may be related to decreased CYP450 hepatic metabolism of the barbiturate.

MANAGEMENT: Patients started on combination therapy should be closely monitored for clinical and laboratory evidence of toxicity. They should be advised to notify their physician if they experience excessive sedation or lethargy.
