MONITOR: Limited data suggest that coadministration of bupropion with valproic acid may increase the plasma concentrations of the active metabolite, hydroxybupropion. The mechanism of interaction has not been established, but may involve inhibition of the metabolism of hydroxybupropion. In five study patients with mood disorders, administration of a single 150 mg dose of bupropion during valproate therapy (mean dose 1750 mg/day for at least 3 weeks) increased mean hydroxybupropion peak plasma concentration (Cmax) and systemic exposure (AUC) by 56% and 94%, respectively, compared to administration with placebo. The clinical significance of these changes is unknown. A case report describes a patient treated with bupropion who developed visual and auditory hallucinations when valproic acid was substituted for lithium. It is unclear whether a pharmacokinetic or pharmacodynamic interaction may have been involved. An increase of nearly 30% in valproate levels following the addition of bupropion has also been reported in one patient.

MANAGEMENT: Until more information is available, serum drug levels and pharmacologic effects should be monitored more closely during coadministration of bupropion and valproic acid. Dosage adjustments may be required if an interaction is suspected.
