Depakote Sprinkles and Mesantoin

Consumer:Using mephenytoin together with divalproex sodium may increase the effects of mephenytoin. Contact your doctor if you experience twitching eye movements, slurred speech, loss of balance, tremor, muscle stiffness or weakness, nausea, vomiting, feeling light-headed, fainting, and slow or shallow breathing. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:MONITOR: Valproic acid and its derivatives may increase the pharmacologic effects of hydantoins. Toxicity may result, despite normal hydantoin levels. Phenytoin is the only hydantoin specifically studied in this regard. The mechanism may be related to displacement of the hydantoin from plasma proteins.
MANAGEMENT: Patients on combination therapy should be closely monitored for clinical and laboratory evidence of altered effects. Patients should be advised to notify their physician if they experience symptoms of toxicity (e.g., drowsiness, visual disturbances, change in mental status, seizures, nausea, or ataxia).
