In a study of eleven patients receiving combination chemotherapy containing irinotecan (a UGT1A1 substrate) with regorafenib 160 mg, mean systemic exposure (AUC) of irinotecan increased by 28% and that of its active metabolite, SN-38, increased by 44% when irinotecan was administered 5 days after the last of 7 daily doses of regorafenib.

MANAGEMENT: Caution may be advisable when regorafenib is used concurrently with medications that are substrates of UGT1A1, particularly those with a narrow therapeutic range. Dosage adjustments as well as clinical and laboratory monitoring may be appropriate for some drugs whenever regorafenib is added to or withdrawn from therapy.
