Diazepam Intensol and Valdecoxib

Using diazePAM together with valdecoxib may increase the effects of diazePAM. Contact your doctor if you experience extreme drowsiness, confusion, muscle weakness, or fainting. You should avoid activities requiring mental alertness until you know how these medications affect you. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with valdecoxib may increase the plasma concentrations of diazepam. The mechanism is valdecoxib inhibition of CYP450 2C19 and 3A4, the isoenzymes responsible for the metabolic clearance of diazepam. According to product labeling, coadministration of valdecoxib (40 mg twice a day) and diazepam (10 mg twice a day) for 12 days resulted in a 28% increase in the systemic exposure (AUC) of diazepam compared to administration of diazepam alone. The pharmacokinetics of valdecoxib were not significantly affected.

MANAGEMENT: Diazepam dosage adjustments should not be necessary during coadministration with valdecoxib. However, the central nervous system-depressant effects of diazepam may be increased, thus patients should be counseled to avoid activities requiring mental alertness until they know how the interaction may affect them.
