The pediatric powder formulation of didanosine should not be used with methadone. Combining these medications can significantly reduce the blood levels of didanosine and possibly make it ineffective. Other formulations of didanosine may be used with methadone. However, you may need more frequent monitoring by your doctor to safely use both medications. GENERALLY AVOID: Coadministration with methadone may significantly decrease the plasma concentrations of didanosine from the pediatric powder formulation. The mechanism of interaction has not been described. In 16 study subjects receiving methadone chronic maintenance, didanosine peak plasma concentration (Cmax) and systemic exposure (AUC) following a single 200 mg dose were 66% and 57% lower, respectively, compared to a parallel control group not receiving methadone. When a 400 mg dose of enteric-coated, delayed-release formulation of didanosine was given to methadone patients, didanosine Cmax and AUC decreased by 16% and 17%, respectively, compared to historical control values reported in healthy subjects.

MANAGEMENT: Given the risk of reduced viral susceptibility and resistance development associated with subtherapeutic antiretroviral drug levels, didanosine pediatric powder should not be prescribed in combination with methadone. If coadministration of didanosine and methadone is necessary, the recommended formulation of didanosine is the enteric-coated, delayed-release formulation (Videx EC). Patients should be closely monitored for adequate clinical response when Videx EC is coadministered with methadone, including monitoring for changes in HIV RNA viral load.
