Diflunisal may decrease the blood levels and effects of sulindac. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Contact your doctor if your symptoms worsen or your condition changes during treatment with these medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration of diflunisal with sulindac may result in a reduction in the therapeutic effects of sulindac. The mechanism for this interaction has not been delineated. In normal volunteers, concomitant use of diflunisal and sulindac resulted in lowering the plasma level of the active sulfide metabolite of sulindac by approximately 33%. Sulindac is a prodrug that requires conversion to the sulfide metabolite, which is responsible for most of the pharmacologic activity. In addition, concomitant use of multiple nonsteroidal anti-inflammatory drugs (NSAIDs) may increase the risk and severity of adverse effects associated with NSAIDs, including but not limited to GI bleeding, GI perforation, and ulcers.

MANAGEMENT: Caution is advised if diflunisal is used concomitantly with sulindac. Dosage adjustments as well as clinical and laboratory monitoring should be considered whenever these drugs are used together.
