Ketoconazole may occasionally increase the blood levels and effects of digoxin. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Contact your doctor if you experience nausea, vomiting, diarrhea, loss of appetite, visual disturbances (blurred vision; light halos around objects; green or yellow vision), or an abnormally fast or slow or uneven heartbeat, as these may be signs and symptoms of excessive digoxin levels. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with ketoconazole may increase the serum concentrations of digoxin. The proposed mechanism is inhibition of the P-glycoprotein-mediated intestinal efflux or renal tubular secretion of digoxin by ketoconazole. Available data are conflicting. In a study of 10 healthy subjects, administration of a single 0.5 mg dose of digoxin following pretreatment with ketoconazole 200 mg orally once a day for 4 days increased digoxin systemic exposure (AUC) by an average of just 9% compared to placebo, which was not significant. However, rare cases of elevated plasma concentrations of digoxin have been reported according to the prescribing information for ketoconazole.

MANAGEMENT: Caution is advised when digoxin is administered with ketoconazole. Serum digoxin levels should be checked regularly and the dosage adjusted accordingly, particularly following initiation or discontinuation of ketoconazole in patients who are stabilized on their digoxin regimen. Patients should be advised to seek medical attention if they experience signs of digoxin toxicity such as nausea, anorexia, visual disturbances, slow pulse, or irregular heartbeat.
