Professional:MONITOR: Coadministration with velpatasvir may increase the serum concentrations of digoxin. The proposed mechanism is velpatasvir inhibition of the P-glycoprotein-mediated intestinal or renal efflux of digoxin. When a single 0.25 mg dose of digoxin was administered with velpatasvir 100 mg to 21 healthy volunteers, mean digoxin peak plasma concentration (Cmax) and systemic exposure (AUC) increased by approximately 1.9- and 1.3-fold, respectively.
MANAGEMENT: Serum digoxin levels and pharmacologic effects should be closely monitored following the addition or withdrawal of sofosbuvir-velpatasvir, and the digoxin dosage adjusted as needed. Patients should be advised to seek medical attention if they experience signs of digoxin toxicity such as nausea, anorexia, visual disturbances, slow pulse, or irregular heartbeat.
