Consumer:

Before taking digoxin, tell your doctor if you also use NIFEdipine. You may need dose adjustments or special tests in order to safely take both medications together. This combination may increase digoxin levels. You should notify your doctor if you have symptoms of nausea, loss of appetite, visual changes, slow pulse, or irregular heartbeats. It is important that you tell your healthcare provider about all other medications that you are using including vitamins and herbs. Do not stop using your medications without first talking to your doctor first.

MONITOR: Limited data suggest that nifedipine may decrease digoxin clearance. Serum digoxin levels and risk of toxicity may be increased. The effect may be dose-dependent, however data are conflicting. Other calcium channel blockers may also interact. Higher doses of nisoldipine may cause small increases in digoxin levels. Digoxin maximum plasma concentrations increased by 33% after 20 mg lercanidipine was given; however, AUC did not change significantly.

MANAGEMENT: Serum digoxin levels should be checked when clinically necessary. Patients should be advised to notify their physician if they experience signs of digoxin toxicity such as nausea, anorexia, visual changes, slow pulse, or irregular heartbeats. If digoxin toxicity is suspected, digoxin dosage should be withheld or reduced.
