Dilantin Infatabs and Nitrofurantoin

In the database of official manuals used in the service creation an interaction registered by statistical results of studies was found, which can either lead to negative consequences for the patient health or strengthen a mutual positive effect. A doctor should be consulted to address the issue of joint drug administration.

Before taking nitrofurantoin, tell your doctor if you also use phenytoin. You may need dose adjustments or special tests in order to safely take both medications together. You should notify your doctor if you have loss of seizure control. It is important that you tell your healthcare provider about all other medications that you are using including vitamins and herbs. Do not stop using your medications without first talking to your doctor first.

MONITOR: One patient experienced seizures and a drop in phenytoin levels while taking concurrent nitrofurantoin. Phenytoin levels rose when nitrofurantoin was discontinued. The mechanism is unknown but may be related to induction of hepatic metabolism by nitrofurantoin. A similar reaction may occur with other hydantoins.

MANAGEMENT: Phenytoin serum concentrations should be monitored during coadministration of these agents. Patients should be advised to promptly report loss of seizure control to their caregiver.
