Before taking amiodarone, tell your doctor if you also use phenytoin. You may need dose adjustments or special tests in order to safely take both medications together. The phenytoin blood levels and amiodarone effectiveness may need evaluated for several weeks after starting therapy. You should notify your doctor if you have symptoms such as drowsiness, visual problems, changes in behavior, nausea, poor muscle coordination, or a worsening of your heart condition. It is important that you tell your healthcare provider about all other medications that you are using including vitamins and herbs. Do not stop using your medications without first talking to your doctor first.

MONITOR: Amiodarone may decrease the hepatic metabolism of hydantoins, resulting in increased serum concentrations. Phenytoin may induce the hepatic metabolism of amiodarone, resulting in decreased serum amiodarone concentrations.

MANAGEMENT: Management consists of monitoring serum hydantoin concentrations and amiodarone effectiveness for several weeks after beginning therapy. Patients should be advised to notify their physician if they experience symptoms of toxicity (e.g., drowsiness, visual disturbances, change in mental status, seizures, nausea, or ataxia) or a worsening of their arrhythmia.
