Rufinamide may increase the levels of phenytoin in your body. If your doctor does prescribe these medications together, you may need a dose adjustment or more frequent blood tests to safely use both medications. Let your doctor know if you experience nausea, vomiting, twitching eye movements, blurred vision, slurred speech, loss of balance, tremors, muscle stiffness, weakness, lightheadedness, fainting, or confusion, as these may be symptoms of excessive effects of phenytoin. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with rufinamide may decrease the clearance and increase the plasma concentrations of phenytoin. The mechanism of interaction is unknown. The decrease in clearance of phenytoin estimated at typical levels of rufinamide is predicted to increase phenytoin plasma levels by 7% to 21%. As phenytoin is known to have nonlinear pharmacokinetics (clearance becomes saturated at higher dosages), it is possible that exposure will be greater than the model prediction. Conversely, phenytoin has been shown to reduce the concentrations of rufinamide by 25% to 46%, and these alterations were not dependent on the dose of phenytoin. Any effects, when they occurred, have been more marked in the pediatric population.

MANAGEMENT: Serum phenytoin levels and pharmacologic response should be monitored more closely whenever rufinamide is added to or withdrawn from therapy, and the phenytoin dosage adjusted as necessary. Patients should be advised to contact their physician if they experience symptoms of phenytoin toxicity such as nausea, vomiting, tremors, ataxia, lethargy, slurred speech, visual disturbances, or changes in mental status.
