Theoretically, the coadministration of rifabutin and methadone may result in decreased plasma concentrations of the latter. The mechanism is accelerated clearance of methadone due to induction of hepatic CYP450 enzymatic activities by rifabutin. Although the interaction is fairly well-established for rifampin, a related agent, conflicting data exist for rifabutin. A study involving 24 patients failed to demonstrate a pharmacokinetic or pharmacodynamic effect of rifabutin on methadone, yet there have been isolated reports of patients who required increased methadone dosages during rifabutin administration due to withdrawal symptoms. These findings may be consistent with the fact that rifabutin is a much less potent CYP450 inducer than rifampin. In general, there does not appear to be a clinically significant interaction between rifabutin and methadone. As a precaution, however, it may be appropriate to monitor the pharmacologic response to methadone and adjust the dosage as necessary, particularly following initiation, discontinuation or change of dosage of rifabutin in patients who are stabilized on their methadone regimen. Patients should be advised to notify their caregiver if they experience possible symptoms of methadone withdrawal, including restlessness, insomnia, sweating, lacrimation, or rhinorrhea.
