Consumer:Using disopyramide together with itraconazole is not recommended. Combining these medications can significantly increase the blood levels of disopyramide. This may increase the risk of serious and potentially life-threatening cardiovascular side effects such as irregular heart rhythm and conduction abnormalities. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:CONTRAINDICATED: Coadministration with potent inhibitors of CYP450 3A4 such as itraconazole may significantly increase the plasma concentrations of disopyramide, which is primarily metabolized by the isoenzyme. High plasma levels of disopyramide may increase the risk of QT interval prolongation, which has been associated with ventricular arrhythmias including torsade de pointes and sudden death. Rarely, azole antifungal agents have also been reported to prolong the QT interval, thus an additive pharmacodynamic effect with disopyramide may occur as well.
MANAGEMENT: Coadministration of disopyramide with itraconazole is considered contraindicated.
