Clearance of single valproate doses increased by 40% in healthy subjects when given after 5 days of rifampin administration. The exact mechanism of interaction has not been established, but induction of valproate metabolism by rifampin is suspected.
MANAGEMENT: Monitoring for altered efficacy is recommended. Patients should be advised to notify their physician if they experience a loss of seizure control or other signs of reduced efficacy. Dosage adjustments may be required if an interaction is suspected.
