Using stiripentol together with divalproex sodium may increase side effects such as drowsiness, dizziness, loss of appetite, nausea, vomiting, and weight loss. You should avoid or limit the use of alcohol during treatment with these medications. Do not engage in activities requiring mental alertness such as driving or operating hazardous machinery until you know how the medications affect you. MONITOR: Coadministration of stiripentol and valproate may increase gastrointestinal adverse effects such as loss of appetite, nausea, vomiting, and weight loss. In two double-blind, placebo-controlled trials in patients with Dravet syndrome receiving valproate and clobazam therapy, loss of appetite occurred in up to 50% and loss of weight in up to 29% of patients in the stiripentol add-on group, compared to 9% and 0% of patients, respectively, in the placebo add-on group. Nausea and vomiting were also reported more frequently in patients receiving stiripentol, with an incidence of up to 25% versus 5% for patients receiving placebo. Plasma concentrations of valproate at the end of the trials were not significantly different from baseline, indicating the lack of a significant pharmacokinetic interaction.

MONITOR: Coadministration of stiripentol and valproate may increase central nervous system adverse effects such as somnolence, dizziness, confusion, difficulty concentrating, and impairment of psychomotor skills.

MANAGEMENT: Patients should be monitored for adverse effects during concomitant treatment with stiripentol and valproate, and dosage adjustments made accordingly if needed. In clinical trials, the daily dosage of valproate could be decreased by approximately 30% per week in patients who experienced gastrointestinal reactions such as anorexia or weight loss. A dietary supplement or increased food intake may also be considered for weight loss. Patients should be advised to avoid or limit consumption of alcohol and to avoid activities requiring mental alertness such as driving or operating hazardous machinery until they know how the medications affect them.
