Consumer:Using rilpivirine together with omeprazole is not recommended. By reducing the acidity level in the stomach, omeprazole can decrease the absorption and blood levels of rilpivirine and make the medication less effective against HIV. Professional:CONTRAINDICATED: Coadministration with proton pump inhibitors may significantly decrease the oral bioavailability of rilpivirine. The proposed mechanism involves a pH-dependent reduction in dissolution and absorption of rilpivirine, which is poorly soluble over a wide pH range. When a single 150 mg dose of rilpivirine was administered with omeprazole 20 mg once daily to 16 study subjects, mean rilpivirine peak plasma concentration (Cmax), systemic exposure (AUC) and trough plasma concentration (Cmin) decreased by 40%, 40% and 33%, respectively, compared to rilpivirine administered alone. No clinically significant changes in omeprazole Cmax and AUC were observed.
MANAGEMENT: Given the risk of reduced viral susceptibility and resistance development associated with subtherapeutic antiretroviral drug levels, concomitant use of rilpivirine with proton pump inhibitors is considered contraindicated.
