Dronedarone and EryPed

In the database of official manuals used in the service creation an interaction registered by statistical results of studies was found, which can either lead to negative consequences for the patient health or strengthen a mutual positive effect. A doctor should be consulted to address the issue of joint drug administration.

Using dronedarone together with erythromycin is not recommended. Erythromycin can increase the blood levels of dronedarone to dangerous levels and cause an irregular heart rhythm that may be serious and potentially life-threatening. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact. You should seek immediate medical attention if you develop sudden dizziness, lightheadedness, fainting, shortness of breath, or heart palpitations during treatment with dronedarone. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

CONTRAINDICATED: Coadministration with potent inhibitors of CYP450 3A4 may significantly increase the plasma concentrations of dronedarone, which is primarily metabolized by the isoenzyme. According to the product labeling for dronedarone, administration of repeated doses of the potent inhibitor ketoconazole led to a 9-fold increase in dronedarone peak plasma concentration (Cmax) and a 17-fold increase in systemic exposure (AUC). Because dronedarone is associated with concentration-dependent prolongation of the QT interval, increased levels may potentiate the risk of ventricular arrhythmias such as torsade de pointes and sudden death.

MANAGEMENT: The use of dronedarone in combination with potent CYP450 3A4 inhibitors such as itraconazole, ketoconazole, voriconazole, nefazodone, cyclosporine, delavirdine, protease inhibitors, and ketolide and certain macrolide antibiotics is considered contraindicated. Patients currently receiving these agents should stop taking them before starting treatment with dronedarone.
