MONITOR: Flecainide suppression of anterograde conduction through an accessory AV pathway was reversed after an isoproterenol infusion, according to one case report. The proposed mechanism is counteractive pharmacologic effects on the AV-conductive tissue. Patients who have underlying arrhythmias may suffer recurrent or new arrhythmias when isoproterenol is added to their medical regimens, even if antiarrhythmic therapy was previously adequate.

MANAGEMENT: Patients should be monitored for altered antiarrhythmic efficacy during concurrent administration.
