Professional:MONITOR: A single case of bradycardia has been associated with the combined use of fentanyl (administered via spinal epidural catheter) and orally administered guanfacine. Although it is possible that bradycardia could have been caused by fentanyl in the thoracic vertebral space, this case may represent a drug-drug interaction. The proposed mechanism is combined pharmacologic effect, as fentanyl and guanfacine can each decrease sympathetic outflow to the heart.
MANAGEMENT: While some centrally acting alpha-antagonists, like guanfacine, are used preoperatively, it is recommended that the clinician monitor for bradycardia and be prepared for its consequences.
