Professional:ADJUST DOSING INTERVAL: Coadministration with H2-receptor antagonists may significantly decrease the oral bioavailability of rilpivirine. The proposed mechanism involves a pH-dependent reduction in dissolution and absorption of rilpivirine, which is poorly soluble over a wide pH range. When a single 40 mg dose of famotidine was administered 2 hours before a single 150 mg dose of rilpivirine to 23 study subjects, mean rilpivirine peak plasma concentration (Cmax) and systemic exposure (AUC) decreased by 85% and 76%, respectively, compared to rilpivirine administered alone. By contrast, when the dose of famotidine was administered either 12 hours before or 4 hours after the rilpivirine dose, no clinically significant changes in rilpivirine Cmax and AUC were observed.
MANAGEMENT: To minimize the risk of reduced viral susceptibility and resistance development associated with subtherapeutic levels of rilpivirine, H2-receptor antagonists should be administered at least 12 hours before or 4 hours after the rilpivirine dose.
