Rifabutin may significantly reduce the blood levels of rilpivirine, which may make it less effective in treating HIV infection. Professional:

ADJUST DOSE: Coadministration with rifabutin may significantly decrease the plasma concentrations of rilpivirine. The proposed mechanism is rifabutin induction of the CYP450 3A4-mediated metabolism of rilpivirine. In 18 study subjects given rilpivirine 25 mg once daily with rifabutin 300 mg once daily, mean rilpivirine peak plasma concentration (Cmax), systemic exposure (AUC) and trough plasma concentration (Cmin) were reduced by 31%, 42% and 48%, respectively. When rilpivirine was given at a higher dosage of 50 mg once daily with rifabutin, mean rilpivirine Cmax and AUC increased by 43% and 16% compared to rilpivirine administered alone at 25 mg once daily, while Cmin decreased by just 7%. The pharmacokinetics of rifabutin and its metabolite, 25-O-desacetylrifabutin, were not significantly altered by rilpivirine.

MANAGEMENT: When used concomitantly with rifabutin, the rilpivirine dosage should be increased from 25 mg once daily to 50 mg once daily, taken with a meal. Following the discontinuation of rifabutin, the rilpivirine dosage should be decreased to 25 mg once daily.
