Using efavirenz together with rifabutin may decrease the effects of rifabutin. Contact your doctor if your condition changes. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

ADJUST DOSE: Coadministration with efavirenz may decrease the plasma concentrations of rifabutin. The mechanism is efavirenz induction of CYP450 3A4, the isoenzyme responsible for the metabolic clearance of rifabutin. In nine study subjects, efavirenz (600 mg once a day for 14 days) decreased the mean steady-state peak plasma concentration (Cmax) and area under the concentration-time curve (AUC) of rifabutin (300 mg once a day for 14 days) by 32% and 38%, respectively, compared to administration of rifabutin alone. Rifabutin had no significant effect on the pharmacokinetics of efavirenz.

MANAGEMENT: Efavirenz labeling and most experts recommend increasing the rifabutin dosage to 450 mg or 600 mg daily, or 600 mg two or three times per week, during coadministration with efavirenz.
