Consumer:Using efavirenz together with posaconazole may increase the effects of efavirenz. Contact your doctor if your condition changes or if you experience increased side effects, such as confusion, lack of balance or coordination, and severe mood or behavior changes. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:GENERALLY AVOID: Coadministration with efavirenz may significantly decrease the plasma concentrations of posaconazole. The proposed mechanism is efavirenz induction of posaconazole metabolism via hepatic UDP-glucuronosyltransferase. According to the product labelling, coadministration of posaconazole suspension (400mg twice a day) with efavirenz (400mg once a day) for 10 and 20 days resulted in reduced posaconazole mean peak plasma concentration (Cmax) by 45% and systemic exposure (AUC) by 50%. Treatment failure could result.
MANAGEMENT: Concomitant use of posaconazole and efavirenz should generally be avoided unless the benefit outweighs the risks.
