Professional:MONITOR: Coadministration with inhibitors of CYP450 3A4 may increase the plasma concentrations of guanfacine, which is primarily metabolized by the isoenzyme. Ketoconazole, a potent CYP450 3A4 inhibitor, has been reported to increase guanfacine peak plasma concentration (Cmax) and systemic exposure (AUC) by approximately 2- and 3-fold, respectively. A computer simulation suggests that fluconazole, a moderate CYP450 3A4 inhibitor, would increase guanfacine Cmax and AUC by about 1.5- and 2-fold, respectively.
MANAGEMENT: Caution is advised when guanfacine is used with CYP450 3A4 inhibitors. Patients should be monitored for increased adverse effects such as hypotension, bradycardia and sedation , and the guanfacine dosage adjusted as necessary.
