Consumer:Using eplerenone together with posaconazole is not recommended. Combining these medications may significantly increase the blood levels and effects of eplerenone. This can increase the risk of serious side effects including hyperkalemia (high blood potassium), which in severe cases can lead to kidney failure, muscle paralysis, irregular heart rhythm, and cardiac arrest. You may be more likely to develop hyperkalemia during treatment with eplerenone if you are elderly, dehydrated, or have kidney disease, diabetes, or advanced heart failure. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:CONTRAINDICATED: Coadministration with potent inhibitors of CYP450 3A4 may significantly increase the plasma concentrations of eplerenone, which is primarily metabolized by the isoenzyme. In a pharmacokinetic study, administration of a single 100 mg dose of eplerenone in combination with the potent CYP450 3A4 inhibitor ketoconazole (200 mg twice a day) resulted in 1.7- and 5.4-fold increases in eplerenone peak plasma concentration (Cmax) and systemic exposure (AUC), respectively.
MANAGEMENT: Concomitant use of eplerenone with potent CYP450 3A4 inhibitors is considered contraindicated.
