Using carBAMazepine together with isoniazid may increase the effects of carBAMazepine. You should report any signs of carBAMazepine toxicity including nausea, visual disturbances, dizziness, or loss of muscle coordination. Using these medications together can also cause liver problems. Contact your doctor if you experience fatigue, weakness, anorexia, nausea, or vomiting. You may need a dose adjustment or special test if you use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Isoniazid increases serum carbamazepine levels in most patients. Also, carbamazepine may potentiate isoniazid hepatotoxicity. The mechanism is CYP450 3A4 inhibition by isoniazid.

MANAGEMENT: Close attention should be paid to clinical and laboratory evidence of hepatotoxicity and carbamazepine toxicity if patients must take isoniazid concurrently. Clinical observation of the patient is recommended, and carbamazepine dosage should be adjusted as indicated. Isoniazid should be discontinued if hepatotoxicity occurs. Patients should be advised to report signs of carbamazepine toxicity (nausea, visual disturbances, dizziness, or ataxia) or early signs of hepatotoxicity (fatigue, weakness, malaise, anorexia, nausea, or vomiting) to their caregiver.
