Before taking erythromycin, tell your doctor if you also use felodipine. You may need dose adjustments or special tests in order to safely take both medications together. The combination may increase felodipine levels. You should notify your doctor if you have symptoms of flushing, ankle edema, or heart palpitations. It is important that you tell your healthcare provider about all other medications that you are using including vitamins and herbs. Do not stop using your medications without first talking to your doctor first.

MONITOR: Coadministration of felodipine and erythromycin may increase felodipine serum concentrations. The presumed mechanism is inhibition of CYP450 3A4 metabolism by erythromycin. Troleandomycin and clarithromycin may also affect felodipine levels.

MANAGEMENT: When macrolide antibiotics are coadministered with felodipine, patients should be monitored for adverse effects such as flushing, ankle edema, or palpitations. Azithromycin and dirithromycin do not inhibit CYP450 and may be considered as alternatives.
