Talk to your doctor before using lithium and spironolactone The combination may increase lithium levels. This combination may cause diarrhea, vomiting, drowsiness, tremor, thirst, lack of coordination, or muscle weakness. If you take both medications together, tell your doctor if you have any of these symptoms. You may need a dose adjustment or special test if you take both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

GENERALLY AVOID: Spironolactone and triamterene may increase serum lithium levels, resulting in a high risk of lithium toxicity. The mechanism is reduced renal clearance of lithium.

MANAGEMENT: Concurrent use of diuretics and lithium should generally be avoided. Extreme caution is recommended if they must be used together. Lithium levels should be monitored daily and lithium dose reductions may be required. Patients should be advised to notify their physician if they experience drowsiness, dizziness, weakness, ataxia, vomiting, diarrhea, tinnitus,tremor, thirst, blurry vision, or increased urination.
