Esomeprazole Capsules and Valium

The proposed mechanism is decreased clearance due to esomeprazole-mediated inhibition of CYP450 2C19. In a study involving epileptic patients, use of esomeprazole (40 mg) resulted in a 13% increase in trough plasma levels of phenytoin. Dose adjustments were not required in this study.

MANAGEMENT: Although a clinically significant interaction is unlikely, caution is advised if esomeprazole is used concomitantly with drugs that are substrates of CYP450 2C19. Clinical and laboratory monitoring may be appropriate whenever esomeprazole is added to or withdrawn from therapy with these drugs. Dosage adjustments may be considered if an interaction is suspected. Patients should be monitored for the development of adverse effects.
