The proposed mechanism is etravirine induction of CYP450 3A4, the isoenzyme responsible for the metabolic clearance of antiarrhythmics such as amiodarone, bepridil, disopyramide, lidocaine, and quinidine.
MANAGEMENT: Caution is advised if etravirine is prescribed to patients receiving antiarrhythmic agents. Drug concentration monitoring is recommended, if available.
