Consumer:

Using pyrimethamine together with methotrexate can increase the risk of side effects that affect your bone marrow function, resulting in low numbers of different types of blood cells. This can make you more likely to develop anemia, bleeding problems, or infections. Contact your doctor if you experience potential signs and symptoms of these conditions such as paleness of skin, fatigue, dizziness, fainting, unusual bleeding or bruising, fever, chills, sore throat, body aches, or other flu-like symptoms. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: Coadministration of pyrimethamine with other antifolate or myelosuppressive agents may have additive effects on rapidly proliferating tissues such as bone marrow, possibly increasing the risk of megaloblastic anemia and other hematologic adverse effects.

MANAGEMENT: Caution is advised if pyrimethamine is prescribed in combination with other antifolate or myelosuppressive agents. Complete blood cell counts should be closely monitored. Treatment interruption or dose reduction may be required if toxicities such as mucositis or myelosuppression develop. Patients should be advised to contact their physician if they develop oral ulcers, painful swallowing, vomiting, diarrhea, and/or signs and symptoms of myelosuppression such as pallor, dizziness, fatigue, lethargy, fainting, easy bruising or bleeding, or signs of infection such as fever, chills, sore throat, body aches, and other influenza-like symptoms.
