Consumer:

Toremifene may increase the effects of warfarin and cause you to bleed more easily. You may need a dose adjustment or more frequent monitoring of your prothrombin time or INR by your doctor to safely use both medications. Call your doctor immediately if you experience any unusual bleeding or bruising, vomiting, blood in your urine or stools, headache, dizziness, or weakness during treatment with these medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: Coadministration with toremifene may increase the plasma concentrations and hypoprothrombinemic effects of warfarin. Toremifene is considered a weak inhibitor of CYP450 2C9, the isoenzyme responsible for the metabolic clearance of the biologically more active S(-) enantiomer of warfarin. The interaction has not been studied nor reported in the medical literature. However, significant increases in INR and serious bleeding complications have been reported in patients receiving warfarin or other coumarin-type anticoagulants in combination with another antiestrogenic agent, tamoxifen, thus a potential pharmacodynamic interaction with toremifene cannot be ruled out.

MANAGEMENT: Caution is advised if warfarin and other similar anticoagulants are used in combination with toremifene. The INR should be checked frequently and anticoagulant dosage adjusted accordingly, particularly following initiation or discontinuation of toremifene in patients who are stabilized on their anticoagulant regimen. Patients should be advised to promptly report any signs of bleeding to their physician, including pain, swelling, headache, dizziness, weakness, prolonged bleeding from cuts, increased menstrual flow, vaginal bleeding, nosebleeds, bleeding of gums from brushing, unusual bleeding or bruising, red or brown urine, or red or black stools.
