Before starting fenoldopam, let your doctor know if you are currently receiving treatment with timolol ophthalmic. Fenoldopam can increase pressure in the eye, which may worsen glaucoma and other eye conditions associated with elevated pressure for which timolol ophthalmic is intended to treat. In addition, since the medication in timolol ophthalmic can be absorbed into the blood, there is a risk of your blood pressure dropping too much when combined with fenoldopam. While fenoldopam alone can occasionally cause excessively low blood pressure, medications like the one in timolol ophthalmic can blunt the body's normal response to keep the blood pressure from getting too low, such as by increasing heart rate. You will need to be closely monitored by your doctor if these medications must be used together. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

GENERALLY AVOID: The use of fenoldopam in combination with beta-blockers has not been studied. Theoretically, beta-blockers may potentiate the risk of severe, symptomatic hypotension by inhibiting the sympathetic reflex response to fenoldopam-induced blood pressure decreases.

MANAGEMENT: Concomitant use of fenoldopam with beta-blockers should be avoided if possible. The same precaution may be applicable to beta-blocker ophthalmic preparations, as they are systemically absorbed and can produce clinically significant systemic effects even at low or undetectable plasma levels. Moreover, since fenoldopam may cause a dose-dependent increase in intraocular pressure, caution should be exercised if it is administered to patients with glaucoma or ocular hypertension receiving ophthalmic beta-blockers.
