Professional:MONITOR: Coadministration with palbociclib may increase the plasma concentrations of drugs that are primarily metabolized by the CYP450 3A4 isoenzyme. Palbociclib has been shown to be a weak time-dependent inhibitor of CYP450 3A4 following daily 125 mg dosing to steady state. In 26 healthy study subjects, administration of midazolam with multiple daily doses of palbociclib resulted in 37% and 61% increases in midazolam peak plasma concentration (Cmax) and systemic exposure (AUC), respectively, compared to midazolam administered alone. The interaction may be significant for sensitive CYP450 3A4 substrates or those that demonstrate a narrow therapeutic index.
MANAGEMENT: Caution is advised when palbociclib is used with sensitive CYP450 3A4 substrates or those that demonstrate a narrow therapeutic index (e.g., ergot alkaloids, alfentanil, cisapride, colchicine, cyclosporine, fentanyl, ivacaftor, macrolide immunosuppressants, oral midazolam, pimozide, quinidine, sufentanil, triazolam, vinca alkaloids). Dosage adjustments as well as clinical and laboratory monitoring may be appropriate whenever palbociclib is added to or withdrawn from therapy.
