Consumer:

Ketoconazole may increase the blood levels and effects of fingolimod. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Contact your doctor if your condition changes or you experience increased side effects of fingolimod such as a slow heart rate, nausea, vomiting, infection, fever, breathing difficulties, and shortness of breath. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with ketoconazole may increase the plasma concentrations of fingolimod and its active metabolite, fingolimod-phosphate. The proposed mechanism is ketoconazole inhibition of fingolimod metabolism via CYP450 4F2 and, to a lesser extent, CYP450 3A4 and 4F12. According to the product labeling, administration of a single 5 mg dose of fingolimod in combination with ketoconazole 200 mg twice daily at steady-state resulted in an increase in the fingolimod and fingolimod-phosphate systemic exposure (AUC) by 70%.

MANAGEMENT: Caution is advised if fingolimod is prescribed in combination with ketoconazole. Patients should be monitored for increased adverse effects of fingolimod such as immunosuppression, bradycardia and/or AV block during initiation of treatment, blood pressure decreases, and respiratory problems.
