Call your doctor if you have symptoms of irregular heartbeat, chest tightness, blurred vision, nausea, and seizures. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

GENERALLY AVOID: Ofloxacin may cause prolongation of the QT interval in some patients. Coadministration with other agents that can prolong the QT interval such as procainamide may result in elevated risk of ventricular arrhythmias, including ventricular tachycardia and torsade de pointes, because of additive arrhythmogenic potential related to their effects on cardiac conduction. Rare cases of torsade de pointes have been reported with ofloxacin alone. In addition, coadministration with ofloxacin may increase the plasma concentrations of procainamide. The proposed mechanism is competitive inhibition of procainamide elimination via active tubular secretion. In nine healthy volunteers, pretreatment with ofloxacin (400 mg orally twice a day for 5 doses) increased the mean peak plasma concentration (Cmax) and area under the concentration-time curve (AUC) of procainamide (1 g single oral dose) by 21% and 27%, respectively, compared to administration of ofloxacin alone. Procainamide plasma clearance decreased by an average of 22%, while renal clearance decreased 30%. Alterations in pharmacodynamic effects of procainamide or the pharmacokinetics of the active n-acetyl metabolite of procainamide were not observed.

MANAGEMENT: The use of ofloxacin in combination with class IA antiarrhythmic agents including procainamide should generally be avoided.
