Using flutamide together with theophylline can increase the effects of theophylline. You should contact your doctor if you experience nausea, vomiting, diarrhea, headache, restlessness, insomnia, seizures, or an irregular heartbeat. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with flutamide may increase the plasma concentrations and the risk of toxicity of theophylline. The proposed mechanism may involve inhibition of CYP450 1A2, which is the principal enzyme responsible for the metabolism of theophylline and the conversion of flutamide to its active metabolite hydroxyflutamide. Patients with chronic obstructive pulmonary disease, congestive heart failure, or cirrhosis may have slower theophylline clearance rates; therefore, they may be at greater risk of developing theophylline toxicity.

MANAGEMENT: Caution is advised if flutamide is used concomitantly with theophylline. Dosage adjustments as well as clinical and laboratory monitoring should be considered whenever flutamide is added to or withdrawn from therapy with theophylline. Patients should be advised to notify their doctor if they experience any signs of theophylline toxicity including nausea, vomiting, diarrhea, headache, restlessness, insomnia, or seizures.
